- Enter your full name exactly as it appears in your patient record.

- Complete payment securely on Stripe.
If you no longer have access to your registered email address, please contact Professor Harvey and his team before making payment.
Privacy: Please do not enter information about your diagnosis, procedure or treatment.
Stripe will email confirmation of the payment to the address entered during checkout. Professor Harvey’s team will reconcile the payment using your registered email address.
